Question : I am in the Bahamas and my 3 year old woke up not feeling so well, symptoms not expressed just lethargic and not himself, then he had bright green diaria around dinner time, and now has the chills and in waking up out of his sleep with abdominal pain and dry heaves. Should I wake him and take him to the hospital here in the Bahamas

Dry heaving,abdominal pain,one episode of altered pattern of stool with lethargy all these may be prodromal symptoms of some intestinal infection(most likely viral).
I would suggest you to give him plenty of fluids,bland diet and observe.For abdominal pain you may give some antispasmodic like dicyclomine with acid reducing agents like lansoprazole or esomeprazole.
5 ml of dicyclomine syrup two to three times a day and lansoprazole tablet (15 mg) once a day will be sufficient for this age.

I am not sure if you will get these over the counter. Please check with a pharmacy nearby. Else visit a primary care provider / pediatrician and get it prescribed.

In the meanwhile if you notice that he gets more lethargic,oral acceptance is poor, decrease in urinary output and increased frequency of stool; consult your doctor immediately.
