Question: I have open sores on the base of my penis and my genitals very painful and they appear to be oozing a clear substance my penis shaft is also red and sore and has been oozing as well but it's now dry.

you may have a sexuality transmitted disease (STD) and is very communicable. Firstly I would ask you to refrain from any sexual activities to prevent sharing infection to your partner. Penile ulcerative lesions are almost always infectious in nature and needs to be treated. Lesions are usually similar in appearance despite differences in causative agents and they need to be tested further in the laboratories to differentiate because treatement varies widely.

Common organisms/diseases are syphilis (treponema) and HSV (herpes simplex). I would suggest seeing a urologist or a STD clinic to get tested and recieve appropriate treatment. More often a tablet will be given rather than an ointment since systemic treatments are more effective for STDs than topical ointments or creams. Antibiotics and antivirals are prescription drugs so I urge you to see a doctor ASAP before your conditions worsens.

The sores are open and not at all like cold sores...so it cannot be herpes right? Also it started from friction during sex. My partner had shaven and it was very rough. I scratched the skin bc it got very itchy?

I'm very sorry for this late reply. Considering your recent disclosure of your recent activity and scratching you may be suffering from contact dermatitis with secondary infection due to scratching. For my patients I would recommend washing the area with a very mild soap (baby soap) while there are lesions and some topical antibiotic would control the infection like mucoporicin or fucidin two-three times a day. Avoid scratching the lesions because it might aggravate your condition. If the condition persists or worsens I would strongly recommend to seek consult with a urologist or a dermatologist so they will be able to conduct a proper physical examaination which is lacking in this forum.
