Question: Hi Doctor, my name XXXXXX , 58 yr I just have a CT Scan Abdomen. And the result is that: - There is possible small gallbladder polyp, extremely enlarged fibroid uterus with IUD in situ. Multiple small renal cysts. anglomyolipoma upper pole right kidney measuring less than 5 mm.. do I need go through Hysterotomic? thank you doctor I need your advice.

Dear Dr XXXXXXX XXXXXXX I already submit my explanation separate with my report.
But I repeat again:
I already have history with multiple fibroid before. I did the CT Scan because I always have painful at the lower side left ( I feel come and out) and discomfort and some time I like feel vomit. In 2007 my gynecologist suggest 2 option because at that time I have a lot of bleeding 1. Hysterectomy 2. Used IUD.
I choice second option until today. My menstruation stop after that.
Hopely it help to answer my question. Looking forward your advise.

See , bleeding is not a problem. So if there is a lot of pain, then you need to treat the fibroids. I have seen the report, and for so many fibroids, and big ones, surgery is the only option. So yes, if the pain is severe and unbearable, you can have the hysterectomy. Otherwise, there is no need for it.

If you decide to have the hysterectomy, the IUD should be removed.
Else, first hormone levels should be checked ( FSH, LH , Estradiol ) to confirm that you have reached menopause, and then , if that is the case, the IUD should be removed.
Yes, the fibroid can cause pressure on the bladder and urethra, leading to urinary problems.
