Question : my husband sometimes out of no where has patches or spots like this on his back what can it be?

Thanks for the query and photograph. It looks like pityriasis versicolor, a fungal infection.
I treat such cases with tablet Fluconazole and an anti fungal cream. And I have seen excellent response with these in my patients. You too can try these in consultation with your doctor.
