What Causes Pressure And Discomfort In Penis After Intercourse?

Question: I visited a massage parlor and received unprotected oral. I immediately had pressure/pain to tip of penis like I have to pee all the time. No other symptoms other than it has been a week and I feel like i have to urinate all the time. Doesn't burn when I pee but pressure/discomfort at tip of penis all the time. I was hopeful it was just a UTI and I had some sulfamethoxazole-TMP at home so have been taking that for 3-4 days and no change at all. I now have lower left abdomen pain from meds or this illness? Constant tip uncomfort and need to urinate. It has been over a week since symptoms started. Thoughts on what it could be and how to stop it? Really don't want to go to Dr if I can help it.I have had oral and vaginal sex w my wife since I returned. Is she at risk?

Thanks for your query.

A. In your post you have not mentioned regarding any past history of systemic illness like Diabetes, Hypertension, Kidney disease, etc.
Under such circumstances, the symptoms can be due to:
1. Urinary Tract Infection (UTI)
2. Sexually Transmitted Disease (STD)
3. Localised trauma to the penis tip

B. Clinical examination is important for diagnosis, so I will advise you to consult your Doctor/ Urologist for a proper clinical examination.
Since you have been experiencing these symptoms for over a week and already taken antibiotics (Sulfamethoxazole-TMP) for 3-4 days without any relief, any further delay in seeking medical attention might be harmful.
Certain investigations like complete blood count, VDRL test, urine examination (microscopy + culture & sensitivity) and Ultrasound scan of abdomen & pelvis will be needed.
They will be helpful in detecting any hidden focus of infection in the Genito urinary tract.
Once the infection is established, a course of antibiotics will be prescribed against the causative organism.
Since you had sexual intercourse with your wife after initiation of symptoms, a prophylactic antibiotic cover needs to be provided to her.

C. Till then, symptomatic relief can be obtained with:
1. Syrup Alkacitron (Alkaline mixture) 10 ml thrice a day to be taken mixed with a glass of water.
2. Drink lots of fluid to maintain hydration status.
3. Avoid acidic food, carbonated drinks, excess tea and coffee.
4. Analgesics like paracetamol, to take care of the pain and associated fever (if any).
5. Maintain proper hygiene.

What STD could it be based on these syphon and the fact the symptoms started almost immediately.

There are more than 30 different bacteria, viruses, and parasites that can cause STDs.
It is difficult to diagnose it only on the basis of symptoms, without performing clinical examination.
The sensation of immediate pain can be attributed to local trauma to the penis tip.
However, persistence of pain over a week associated with abdominal pain suggests the diagnosis towards a super-added UTI or STD.
Though symptomatic relief can be obtained with the measures mentioned in my previous answer, a proper clinical examination and investigations need to be performed for clinching the diagnosis.

There is no need to worry, you will be fine.
