Hello

I am writing regarding the sinus condition i wrote about before.

Unfortunately it is not gotten any better at all, and frankly the four months I have spent in the dry air of Canada have been hell.

To review the most predominant symptoms:

-pain/pressure behind the nose not diminished...i need to emphasize the feeling of it being plugged, especially when really bad (in the morning and late afternoon usually..and especially indoors at my mom's place) and the "rattling" around i hear and feel as i tilt my head to the side back and forth.

-constantly clearing throat, sometimes up to 20-30 times a day

-very sensitive under cheek bones (had this for years)

- as pain increases, often the nostrils contract as i breathe in, though only when bad and the nasal passages themselves, while dry are usually clear

- pain often, especially in morning, at side of forehead as well. i use Icyhot for this

-slightly sore upper back throat, more than in past

-steam usually helps a little

-spicy food really helps, but usually only for 20 minutes or so and right after i often have to clear my throat a lot



In 6 days i return to the humidity of Panama where it is usually much much better, though never really goes away and it can bother me there too. and of course i need to come back to canada so i NEED to get better.

(i read the CT scan report and what it seems is that they can't find anything specifically wrong..which drives me crazy and is the reason that the ENT here wouldn't see me)



remember the mucous is always clear - though once in awhile a "chunk" of white mucous comes out (though not often)..so it is not an infection.



off and on i have had this for almost 20 years. but the last 4 months have been the worst by far in years (since i arrived to canada from panama...i suspect my mom's place is very dry)



None of the medications seemed to help much at all, except the steam and saline rinse a little



as you said i think maybe sinuplasty is the best option and i would like to pursue that in panama with an ENT at one of their better hospitals.

Detailed Answer:
Hi,

Thank you for writing back.

1. The microdicom download worked. I have reviewed the CT images.

2. The CT scan is near normal, with normal anatomical variants. Mild maxillary sinusitis and turbinate hypertrophy is seen. The source of the mucus does not seem to be the sinuses, but from the nasal obstruction.

3. For chronic nasal obstruction, I get a post decongestion CT PNS (Para Nasal Sinuses) done and compare the images with the improvement in the symptoms 15 minutes after the instillation of 5-8 drops of Ortivin (0.1% Xylometazoline) nasal drops.

4. More than Balloon Sinuplasty, Radio-frequency turbinate reduction should help. In the scan the right side seems to be more obstructed. This would vary with the. nasal cycle. RF will open the nasal valve area and the posterior ends of the inferior nasal turbinates which seem to be causing the feeling of obstruction behind the nose.

5. A sea mineral hypertonic nasal spray (such as Physiomer) should be tried first. Acid reflux will worsen a post nasal drip and vice versa.
