Question: Pressure on diaphragm and swollen abd causing me breathing difficulty and could this also be liver?

Yes this can be a sign of liver disease.
But since you suffer from heart disease it can be due to Congestive Heart Failure which can also cause ascites leading to abdominal discomfort and breathlessness.
Provide me following details:
1. Since when are you having this?
2. Any chest pain?
3. Any breathlessness on lying down position?
4. Any cough?
5. Any pain abdomen?
6. Any fever?
7. Any jaundice?
8. Any swelling on legs?
Revert with these details for a better help.

Yes,I DI HAVE CHF, but I have just recently developed swollen abdomen and increased sob. With any activity I grunt and gasp and am feeling pressure ondiaphram.

With the current Details available you seem to be having Decompensated Heart Failure that is causing your current problems.
I would advise you to limit the intake of salt and water and seek help.
Get following:
1. Echocardiography.
2. ECG
3. Xray Chest
4. Ultrasound Abdomen
5. Liver and Kidney function test.
These tests are required on urgent basis. There is no point in waiting.
Infact if you have severe breathlessness you should visit ER now.
