Brief Answer:
Likely benign, no need to worry

Detailed Answer:
Hello,
Thank you for trusting HealthcareMagic

Painless lump behind ear could be sebaceous cyst, lol ima, benign tumors or bony protrusion.
Sebaceous cyst and lipoma will be soft in consistency.
The latter two will be hard and bony consistency.
Physical examination is needed to confirm the diagnosis.
Since you have been having the symptoms for a long time now, it is likely benign and nothing to be worried about.
If you notice increase in size, pain, redness or discharge from area, consult a primary care provider for further evaluation.
