Based on your description, its possible that the protrusion is related to an enlarged node related to a local infection leading to submandibular gland enlargement. The fact that there is pain associated with it especially if on movement then TMJ disorder is also a possibility, in which case muscle relaxants and NSAIDs can be advised accordingly. The jaw and oral cavity esp teeth needs to be examined for any associated focus and treated accordingly.
