What Causes Right-sided Flank Pain And Hematuria?

Question: 55yo wF, right back flank horrible pain. Hematuria (microscopic). Went to ER -posterior ct scan showed no renal calculi. But pain is off the charts. Referred me to urologist. Hard to find on on weekends. Thoughts on diagnosis?

Thanks for your query,based on the facts that you have posted it appears that you have microscopic Hematuria and severe pain in Rt flank

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have High B.P.?
2) Are you taking any drugs especially Aspirin?
3) Did you have vomiting?
4) Do you have any other Urinary Symptoms like urgency,burning ,frequency etc?

The cause of severe pain in back on Rt side with microscopic hematuria could be due to small calculus which has passed recently before CT scan was performed hence not detected on CT findings.

Please get Routine Urine examination and urine culture done and start taking broad spectrum antibiotics like Cefexime along with urinary antiseptic like Nitrofurantoin and pain killers like Brufen twice daily and urine alkaliser thrice daily.Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water to keep your urine dilute .

Please consult qualified Urologist for clinical evaluation if pain continues or recurs again.

No other urinary symptoms. No HTN. ASA 81 mg/day for atrial fib prophylaxis. Clotting labs normal last night in ER.

Since you have microscopic hematuria (Not gross hematuria)you can certainly observe for 5 days and repeat urine examination .

If pain continues consult qualified Urologist for further evaluation and tests like Intra Venous Pyelography (I.V.P).

Continue taking medications suggested in my reply earlier.
