Two Abortions, Oligohydramnios, Kidneys Not Visualized, Next Scan Showed No Heartbeat. Precaution To Be Taken For Next Time?

Good morning doctor
I had two abortion -1 at 2nd month of pregnancy and other at 26th week of pregnancy.1st case-large sub chorionic haematoma.2nd oligohydraminos-IUFD-following medical abortion .took US scan at 7 weeks,8 weeks,20 weeks and 25 weeks.in 20 weeks US scan there was reduced growth of 19 days.doctor said -no problem.prescribed only iron and ASA tablets.but when us scan taken at 25th week -found disparity of 5 weeks and oligohydramnios ,kidneys are not well visualized. admitted in the hospital and took LONOPIN injection 40ml
[low molecular weight heparin} for 5 days -Two inj.per day.8th day -us scan -found the heat beat of foetus stopped and undergone abortion . doctor pls give me an answer------
Why this happened ? What all precautions are to be taken at next time ?

Oligohydramnios & non-visualization of kidneys may indicate presence of a genetic or congenital abnormality which is not avoidable. Nothing can be said about the cause of your first miscarriage, although approximately 50% of pregnancies that result in miscarriage during first trimester are genetically abnormal.
This speculation can be confirmed if the fetus or products of conception are studied in lab ( genetic testing or karyotyping). This can also give you some idea about the potential of recurrence.
If this test was not done before, you (both partners) should also have karyotyping.
If in fact genetic abnormality turned out to be the definite cause, you can't do much about it ( apart from options of gamete / egg donations or adoptions. Otherwise, if no genetic cause then low dose Aspirin & LMWH ( low molecular weight heparin) may do miracles in future pregnancies.
Better is to consult a good Gynecologist for full investigation & future help.
