What Causes Right-sided Pinching Pain In Chest?

Question: Hi - Today I was doing some cardio exercise on an exercise bike - aiming for a target heart-rate of about 152. After about 27 minutes I had a pinching pain on the right side of my chest, It seems to go away after about 5 seconds. I continued to bike and take my heart up to 160BPM for several minutes. No problems.

Can Angina be variant like this or is this most likely just something else not to worry about.

Not shortness of breath, other than what would be expected when exercising (heavy breathing). I had a stress-test about 4 years ago that was normal.

Hey there.. Welcome to HCM.
I have read your question and understand your concerns.
The symptoms you have described are less likely to be a cardiac symptom. The symptoms of heart patient on trademill are heaviness and pain of center or left side of chest and uneasiness. The symptoms will be persistent usually everytime while reaching target heart rate level during stress test.

Usually during stress test we expect only breathlessness during peak exercise. Sometimes nonspecific chest pain may occur which should be accociated with significant ecg changes to call it Positive trademill test. Otherwise it has no significance. I have seen in my patients with significant blockage who usually have significant symptoms within first 5 minutes only of starting the test. So you don't need to worry.

So, my advice advice to you is -

1. Go for STRESS ECHO STUDY which is more specific then routine stress test in diagnosing any blockade in the coronary arteries. One more advantage is your echocardiography will also be done simultaneously which can reveal other structural abnormalities if any (like MVP - Mitral valve prolapse) which can sometimes give rise to nonspecific pain.

2.If stress echo study is not possible due to any reason ask your doctor to do routine echo at least and stress test.

Don't worry dear everything will be come out to be normal only. I have seen your medical history. You feel occasional chest pain and worried most of time. So don't think much and go for investigations I have advised.

CT coronary Angio is also an good alternative to rule out once and for all any significant blockade or not but we will think about it later.
