What Causes Severe Pain Behind The Knees While Folding The Leg?

Question: Hello Doctor,
From couple of days, my husband is complaining of pain behind right knee and if he folds his leg, he feels a pain in specific line starting from top of the heel till back of his thighs. His both heels also pain at a very specific spot. His body gets stiff/heavy in the mornings when we sleep with fan on for entire night. Are these problems related to some sort of arthritis? Awaiting response, thanks in advance.

Good evening there

Well, the sort of symptoms you are telling looks like it could be a start of arthritis ( maybe an acute one or chronic, we dont know yet). Since he is not able to fold his knee, I am suspecting there is knee swelling either on the sides or the back causing limitation of the movement. I would like to ask you a few more queries before guiding you further:

1. Does he have back pain or neck pain?

2. Does he have stiffness in the mornings in the back??

3. Any difficulty in turning position at night?

4. Anyone in the family having similar symptoms???

Hello Dr Naval,
Thanks for a quick reply. His pains are still bearable and he can definitely fold his legs, like yesterday he sat folding his legs for 2 hours but it pained the moment he stood up and the pain remained till next one hour.

1. Does he have back pain or neck pain? Yes, neck pains only in morning

2. Does he have stiffness in the mornings in the back? Yes

3. Any difficulty in turning position at night? No

4. Anyone in the family having similar symptoms? Mother has arthritis since last one year (she is 61 now) and also has Sciatica

Sorry for the late reply. Was caught up in some work

Well, the symptoms you have quoted does look like pointing towards Arthritis , in the category of Spondyloarthopathy.

To summarize:

1. Knee pain
2. B/L heel pain--> Probable plantar fascitis
3. EArly morning stiffness in the back along with neck pain

These symptoms go towards an inflammatory arthritis--> Spodyloarthopathy( you can read on net www.rheumatology.org--> Patient education--> Spondyloarthopathy Examining the patient still remains the best way out to determine where we are heading with it.

I would request you to get a few blood tests done and consult a rheumatologist near you

1. ESR
2. CRP
3. HLA B 27( PCR)
4. x Ray pelvis AP view

For treatment, give him some pain killers and see how he feels

Good evening there

Well, it is not curable, but controllable. But would like to confirm the diagnosis first before we can give you the future course. Examinations are helpful in these cases.

Mostly we recommend regular exercises for it, which will help loosen up his body and get rid of the back pain. Secondly , get him to wear sports shoes most of the times as it will help the feet pain.

In XXXXXXX you can consult Dr XXXXXXX Chopra. He practises on MG road, ( not sure about the appartment, but it is called harmen appartment or something. He is quite XXXXXXX and will look into your case.

You can update me with the reports when it is done as well. Meanwhile you can start him on Tablet Nucoxia 90 mg once daily( if no allergies to the drug previously)
