I will recommend you continue with the Motrin and refrain from trying to lift very heavy objects in future. It would be advisable to carry with someone or to divide into smaller sizes and carry many times if possible.
Motrin can cause stomach pain. I will advise that you take it with food. If the pain persist being 5days, I will recommend you reduce the dose to 400 mg and add tylenol for better pain control and to prevent possible side effects(gastritis).

With a severely swollen testicle, this is more concerning. Could it have been a hernia? I cannot answer these questions without a a good examination. I will suggest you make use of the ER at this time so that the exact problem can be addressed.
For now,, continue with the pain medications and put on a supportive underwear to reduce the pains caused by the weight of the testes by providing a surface on which them can be supported.
