What Causes Tingling Sensation In Limbs, Tinnitus And Muscle Twitches In Body?

Question: Hello sir/madam,
I am suffering from vague neurological symptoms. From three years i am having tingling sensations in all limbs but only when i apply little pressure(even when carry small grocery bag then i get tingling in fingers.) Everybody gets the sensations when they sit cross legged for long time but in my case it starts within fraction of seconds. Before these symptoms started i had hair loss and insomnia. I visited a neurologist. He had done nerve conduction tests, B12, Vit D, Diabetes,CBP,Thyroid profile.All these tests came normal.BP was normal.He prescribed Pregabalin but no use. Then Doctor had done MRI Brain and MRI Spine. Every thing came normal. Then the doctor said that the symptoms are because of anxiety. I didn't believe it. i had been prescribed Duloxetine. I used this for six months. I started thinking about Multiple sclerosis. Thinking about MS made me sleepless.After one year day long giddiness had been started. I visited another neurologist. He had done tests like ANA,CPK,CBP tests also he has done Nerve Conduction Tests for the second time.
He prescribed Pregabalin 75 the giddiness gone away. Later many symptoms have been developed they are:
1. Tinnitis in both the ears(visited ENT Specialist but hearing is normal)
2. slow pitch tremors in all limbs(I used INDEROL but no use, after some day have stopped)
3. Muscle twitches in many muscle of my body (particularly twitches in all four eye lids.).They get better after some days
Now from one month back i am having weakness in right leg. The Doctor examined and it is just Subjective weakness and everything is normal and prescribed B12 and Nexitoplus. But there is no used and even the severe giddinedd restarted now. At present i have following symptoms:
1. Tingling in all limbs when apply little pressure/ when i cross legged for little time.
2. Severe tinnitus in both the ears.(even without hearing loud sounds)
3. Having feeling like i have some insect in the ears when i hear loud sounds.
4. Day long giddiness/the feeling is at the back of skull.
5. Right leg weakness(if i walk for small distances i am having fatigue or pain in the leg)(Doctor said it is subjective weakness)
note: My family history is good. nobody had any chronic.neurological disease.
Sir please tell me what is causing all the symptoms? Is it Multiple scelorisis or any other disease? Please help me in diagnosis.

I have gone through your question and understand your concerns. You have been evaluated by doctor and gone through multiple tests with normal reports.
First concern is regarding multiple sclerosis. As there is no objective evidence of visual loss, focal neurological deficits or Mri finding the possibility is less.
The second concern about vertigo with tinnitus may be due to meneire disease if associated fullness of ear and decreased hearing is present.
If all reports are normal then generalized body pain may be due to fibromyalgia
You can try Flunarizine with amitryptiline with some vertigo exercises to releive your symptoms.
Nothing serious at present.

but its not vertigo.its giddiness and abnormal feeling in all the limbs.
I get tested by ENT he said there is no hearing loss and no meneire disease.
My major concern is tingling when apply small amount of pressure ie, even if carry a carry bag.

Such a tingling sensation with pressure is not of any neurological significance if it is present all over the body.
Some times in particular nerve distribution, pressure may cause tingling known as tinel sign.
I will advise you to be in touch with your doctor and if some objective deficit or signs occurs then be investigated accordingly.
At present nothing of much concern.

Thank you for your kind answer.
As I said i am not having vertigo but only lightheadedness. Is lightheadedness caused by any ear problem? If it so, can you suggest me any medicine for cure/management of lightheadedness/ear problem?
And my another problem which is making me anxious/think about MS is having subjective right leg weakness. It disturbing me much when i walk for small distances(an awkward feeling and little pain in right foot when i walk). Will you suggest any medicine for this subjective right leg weakness? or should i ignore it?
My doctor was saying that tingling when applying pressure is due to nerve sensitivity. But did not understand what does his mean by nerve sensitivity in spite of having normal nerve conduction. And he said that all other symptoms i mentioned in the first question/query are just Psychosomatic symptoms which are due to constant anxiety. Is it true that having this many physical problems due to anxiety?
Now i am not so anxious about MS but but i want to solve the following two issues.
1. Tinnitus(bilateral) cum lightheadedness(daylong)
2. subjective right leg weakness.
Kindly suggest any solution for these two problems.

I have gone through your question and understand your position.
Firstly tinnitus and light headedness have many causes but local ear cause is the commonest. You have normal blood pressure? For treatment you can try Flunarizine or cinnarizine. Some vertigo exercises may also help.
Second issue regarding right lower limb weakness is to be better evaluated by a neurologist clinically and image lumbosacral spine region for any disc protrusion. Lesion of multiple sclerosis usually resolve in few day. As you have me both brain and spine Mri are normal, MS is less likely.
Anxiety can lead to increased symptoms.
You need to do regular exercise, take fruits and plenty of water, proper sleep.
You can continue pregabalin and duloxetine and doses can be increased.
One investigation I will advise you is serum Vit D3 levels. You can consult me again after the report via putting Direct Question to me.

Good evening sir,thank u very much for your patient answers for my queries.

Just before 15 days i get tested for B12 and VitD. Both came normal(B12-677pg/mL, vitD-69 ng/mL) and i need to mention that i am having above mentioned problems before these tests get tested.
My BP is fluctuating in each testing.Sometimes normal and sometimes reaching 140/90.Usually high in the morning.

The main problem for me is that i am not able to control thoughts about MS. Actually when the problem started(3 years back), i had only one problem that is tingling by applying little pressure all over the body. when i started thinking about the serious disease like MS all other problems had been started.

Now epicenter of my anxiety is thinking about MS. I am thinking that i had MRI of Brain and Spine before 2 and 1/2 years and at that point of time i had only one problem that is tingling by apply little pressure. Now i have multiple somatic symptoms so i am thinking that all these problems are due to MS and I need to get MRI's one more time. But my doctor is saying that no need of taking MRI's once more. I am not understanding why he is saying like that.

Can you suggest me, is it not necessary to the MRI's of Brain and Spine one more time?
I am thinking always that lightheadedness and Subjective right leg weakness is due to MS. But my doctor after testing saying that it is not at all MS.

I can understand your concern regarding multiple sclerosis.
It is usually a waxing waning disease with symptoms appearing and improving near to normal. Vision involvement in form of optic neuritis is one of the commonest features though cases of clinically isolated syndrome characterised by only spinal cord involvement may also be a presentation.
It's necessary by clinician to evaluate properly regarding any evidence of optic neuritis or brain or spinal cord involvement.
In your case, you can take a second opinion by another neurologist so as to rule out any organic involvement.
Mri positivity in absence of symptoms is usually uncommon and is not a recommendation for treatment.
Be calm and avoid stress.
