What Causes Vomiting, Severe Diarrhea And Lower Abdominal Pain?

Question: My 7 year old daughter has been throwing up for 3 days. She also had sever diarrhea the 1st day. She has left lower abdominal pain and has been throwing up dark brown/black flakes. She can't keep ANYTHING down...she throws it right up. She went to her pediatrician today who prescribed her suppository nauseau medicine which so far has not helped...just makes her sleepy and also referred us to a GI specialist. Her appointment is tomorrow with the GI specialist at 1pm. She is very pale, does not look well and is in pain in her lower left abdomen. Can you tell me what it cohld possibly be? Is this something I should bring her to the ER tonight or wait until her appointment with the GI specialist tomorrow at 1pm?

Also, she has been having on-going stomach problems like this for the past year. Originally she had an xray done which was fine just ahowed some gas bubbles. So we thought she was lactose intolerant. But she was still having some episodes (diarrhea/vomitting) so I introduced milk back into her diet and she has tolerated it just fine. This was 9 months ago.
Now the past few weeks her episodes are becoming more frequent (usually they last 1 full day) but this time it is MUCH MUCH worse....and she has never complained of her lower left abdomen hurting until now. I can't even press on it...it hurts her too much.

I would really appreciate your thought and advice.....

Thanks for the query

I can understand your concern.
She needs an emergency management and then some investigations to find out the cause leading to this condition.
Due to repeated vomiting there will be fluid loss and she may get dehydrated.As she has already taken suppository and that didn't work;the first thing is to control the vomiting with help on antiemetic(like ondensetron) injection.
If she is lethargic,eyes appears to be shunken and the urinary frequency has decreased;she may need to be hydrated with IV fluids.Once the vomiting stops oral antiemetic and oral fluids will help.

Recurrent history of diarrhea and vomiting can be due to repeated infection or some chronic malabsorption condition.I would like to get a celiac serology and stool test done.Also an USG abdomen/CT scan should be done to see whether there is any sign of colitis(thickness of bowel loops) causing pain.
I would like to know if there is any history of blood mixed stool or any family history of GI disorder.
If all these tests are inconclusive a capsule endoscopy may be needed to see the inside of intestine.
