Brief Answer:
May be Fungal infection.to rule out phimosis.

Detailed Answer:
Hi
Thank you for query. I am Dr Bharatesh D Basti answering your query.
It is not clear about the exact morphology, site ,duration ,symptoms, other symptoms and disease, treatment taken, partner history whether she has white discharge or itching or fungal infection etc.
It may be due to fungal infection or balano posthitis.
As you are having problems of retracting the fore skin and pain during intercourse.it is better you consult surgeon for examination if you are going for Phimosis which requires minor procedure like slitting or circumcision.
Clean the area with warm water, twice daily.apply local antifungal creams like clotrimazole twice daily.keep area dry and without friction.avoid intercourse.or with condoms only for now.
If partner is having discharge treat her also with topical antifungal.Oral antifungal may be necessary for both.
You can get back with details and photos for further clarifications.
Thank you
Dr Bharatesh

Brief Answer:
Likely Fungal Infection going for phimosis.

Detailed Answer:
Hi
Thank you for follow up query.
it appears by photo that You have fungal infection and going for phimosis as it appears you are unable to retract.
Please take oral antifungals like flucanazole 150mg weekly once 4 weeks along with other measures and topical suggestions given earlier.Treat your wife also with the same.
avoid intercourse for now.
Maintain genital hygiene.
You can consult surgeon if retraction is a problem as you are going for Phimosis which requires minor procedure like slitting or circumcision soon.otherwise Later it might impair your intercourse.
You can get back to me for any clarifications else please close the discussion and rate.
Thank you
Dr Bharatesh
