Question: The hospital have conducted a PT Time and INR blood the results PT 13 seconds and INR 1.1 is this acceptable,,, no medication at all.... Patient immobile about 6 months

Based on your query, my opinion is as follows:

1. The PT time is well within the normal range of 10-14 seconds.

2. INR ratio is usually calculated in patients, who are on anti-coagulation therapy. As the patient is not on any therapy, 1.1 is normal.

3. Complete immobility increases risk of deep vein thrombosis and pulmonary embolism. If she is completely immobile, then anti-coagulation therapy maybe necessary.

You need to discuss with your doctor regarding risk of thrombosis and anti-coagulation therapy. At present both PT and INR are within normal range.

The legs are significantly swollen...and assessed as high medical VTE...what need to be done in view of 6 month immobility and inr of 1.1.

High medication VTE means an increased risk of embolism based on thrombus formation in leg. This has given rise to swollen leg. This thrombus formation is due to long time immobility.

Thrombolysis or removal of thrombus from the leg veins is necessary. Initially a Doppler study to identify the number and size of thrombus is essential. Then only surgery will be necessary. If a thrombus gets dislodged and moves to lung, risk of pulmonary embolism along with respiratory failure is possible.

Not sure about present physical condition for surgery - without surgery - difficult to treat VTE.

Also, anticoagulation therapy is necessary at the earliest to avoid further risk of thrombosis. After anticoagulation therapy, INR of 2.5-3 needs to be maintained.

Discuss with your doctor regarding tests and further treatment.
