What Do The Following Test Reports Indicate?

Question: Hello doctor,

I am 31 years old and have been married for 2 years, we are trying to have a baby from past 1.5 years. Attached are the mentioned reports and summary below.

1) I had visited Dr XXXXXXX XXXXXXX for the month of July and August 2015 attached are the details of the folicle scan and medications prescribed by her ( file name Bhargavi). I changed the doctor since was not happy with the way junior doctors behaved and very rude and unprofessional.

2) I started consulting Dr. shoba XXXXXXX GYNO, from September she prescribed MCBM 69 every night and consevel twice a day, morning and night for 3 months. My LMP was 9th September 2014

I had the following tests done on day 3 of my September LMP, the tests are CBC, glucose fasting, testosterone, TSH, Free T4, FSH, LH AN Prolactin, SHBG, AMH, 25 hydro vitamin D and as per reports I had a vitamin D defiency and was asked to take Dr. Solution 60,000 IU weekly with milk for 8 weeks and the 5 sachets once a month for 5 months. (all test reports and scan reports and doctor prescription attached file name SV)

Due to over/change in medication i had a cyst formation and doctor advice me take lactre every night from my 3rd day of my LMP for 21 days in month of October 2014 for 21 days.

In November 2014 my LMP was 8th November and I had a natural cycle with out medications and my follicle growth was not good and had no ovulation.

In month of December 2014 my LMP was 10th December 2014 and was adviced to take CCQ50 from day2 to day 6, 1 tablet every night each day and had folicullar scan from day 9 till the follicle size was =>18*18 once the size was achieved, I had a injection Fertgyn 5000 IU and the follicle ruputered. I was given duphaston for 15 days 2 tablets morning and night and We were asked to have relations on alternative days that whole week.

In addition I was deducted with PCOS in the same month in my scan prescribed me Fertripro sachets every night, and glycomet twice a day morning and night..

Same medications and procedure continued for the month of January and February 2015.
I had good follicular growth and ruptured with injection.

I have also attached my husband semen analysis report as on September 2014 and latest on as of march 2015, he was prescribed paternia for 3 months after the 1st semen analysis report

Since I did not conceive so doctor asked me to under go HSG test in month of march and I had the test on day seven the reports are normal. After seeing my reports doctor asked me to meet fertility specialist in could nine Dr. XXXXXXX nelivigi.

In the month of April my LMP was on 13 april 2015 on day 2 the doctor had a scan, she prescribed me to have Ubiphene n for 5 days every night and 3 Gonal injection F 75 IU on 17th, 18, and 19th of april 2015 and day 10 the follicular scan showed one egg of 1.7cm *1.8cm in RO and multiple eggs the LO.
After the following scan I was adviced to take injection Leupride 1mg on 23rd april 9 PM and had a IUI procedure on 25th April 2015 around 10.30-11 pm
She prescribed capsule puregest 400mg vaginally every night for 15 days and duphaston twice for 15 days and asked me to do a pregnancy test after 15 days which I performed today in the morning and turned out as negative and I got into my periods today (all test reports and scan reports and doctor prescription attached file name LN)


My question are a below,

-     Please advice if IUI is a right procedure for me and what are my chances of pregnancy after analysis of my reports as I got in to my monthly periods today

-     If possible pls advice the best fertility specialist in XXXXXXX

-     I have PCOs with regular periods main problem is with follicle growth and ovulation. How difficult is it to get pregnant with these issue

-     I had a good egg growth will medicine and ovulation triggered by injection; Can I have a good quality of egg after ovulation with these medicines?

-     I there any other reason I am not able to get pregnant?

Now regarding your question.
1. Yes, your husband's semen count is not optimum, so IUI is the right procedure for you.
PRegnancy rates in IUI vary from 10 - 15 %, so those are the chances of success for you in best hands.
Maximum, you can go for 3 - 4 more trials of IUI, before considering the next step.
The next step is to go for a laparoscopy to check patency of fallopian tubes ( HSG is not always conclusive ).
Then you can consider IVF if all else fails or if tubes are blocked.
2. Cloudnine is the best centre in XXXXXXX and you can definitely continue treatment there.
3. Women with PCOS often have a problem in ovulation, so drugs and injections are often required for ovulation, so yes, do not worry, with medication, you can definitely ovulate and have good chances of pregnancy.
4. What is your weight ? I think the problem is PCOS and slightly sub optimal semen count. But do not lose hope, please try for a few more cycles of IUI, I have explained the success rate to you, so you need to keep trying.
Take regular folic acid supplements, and exercise daily.
Avoid stress.
Do not lose hope , you are young and can definitely conceive.

Thanks a lot for your reply Dr. XXXXXXX My weight was consistent between 47.80 to 48. 20kg's from past 6 month but in month of april after having the Gonal 75 IU injection for 3 days i almost put on weight immediately by 1+kg now i am 49.5kg.

pls advice even after having HSG test can i still have blocked tubes or fiberiods? as IUI will not work in case if i have blocked tubes right?

With regard to my husband semen count the first report done during september had less count but after taking paternia tablets for 3 month he had a semen test again in march 2015 and the count had changed and improved a lot, I had attached both the reports in SV file.. Please advice if here is any more improvement required.

I had a query on Puregest 500mg tablet i was asked to use it vaginally every night for 15 days after IUI procedure, but when i used to insert it vaginally in night, in the morning i used to have the white thick liquid in my panties. Does this mean the medicine was not taken properly?

You do not seem to be overweight at all ( what is your height by the way ) ?
SO that is fine.
Continue exercising / meditation - that will always help.
If HSG shows open tubes, generally they are open.
However, laparoscopy helps to pick up many other conditions - fibroids compressing the wall of the uterus, endometriosis, ovarian cysts, fimbrial cysts , polyps etc - that are often missed on HSG, and can contribute to difficulty in conception.
Hence if 3 more cycles of IUI fail, a laparoscopy can be done before IVF is considered for you.
Yes, the latest semen analysis done in March 2015 is quite normal in terms of count and motility, so it is fine and quite acceptable.
In fact the whitish liquid on the panties means that the medication worked properly.
This is because these vaginal tablets get dissolved and the whitish substance comes out of the vagina when the medication dissolves and gets absorbed.
Only one thing is it should be inserted high up, atleast 2 out of the 3 knuckes of your finger should be inside the vagina while inserting the tablet.
