Question : Hello Doctor, I had reached out for your suggestion 2 months back as my husband's triglycerides and cholosterol are high. After diet changes also there is no improvement in numbers. Can you check the latest reports and suggest next steps please. Thank you very much.

Thanks for your query.

I have seen your current and previous reports. There is not much change in cholesterol levels.

According to international guidelines, lipid lowering drugs are to be taken if LDL is more than 190. In your husbands case it is less than 190. Triglycerides are somewhat raised. In XXXXXXX there is a trend to give medicines if triglycerides are also raised.

I would still suggest that food be free of milk or milk products like cheese, butter, ghee, sweets, chocolates etc. Sine he is not having any health condition like hypertension or diabetes, I think he does not require medicine yet.

Medicine like tablet atorvastatin may be taken at dose of 10 mg OD at night for 4 weeks after consulting with your doctor.

Thank you doctor for the advice. shall we continue to watch our diet and see if the numbers improve? Any other dietary changes and health routine that you would prescribe without tablets

Hi again

In absence of any additional medical conditions like high blood pressure and diabetes, there is no harm in continuing lifestyle changes alone for next three months. Recheck the lipid profile after 3 months time.

The lifestyle changes should include:

-Diet changes need to be done as the fatty diet is the usual culprit. Reduced intake of saturated fats to less than 7% and cholesterol to less than 200 mg/day. This means to completely exclude milk products like ghee, butter, cream, ice cream, sweets, chocolate, high fat milk, cookies, cakes, pastries. Non veg food is also to be avoided. If it is must to take it, then the fat has to stripped nicely from it. Take 3 smaller meals per day.

-increase the soluble fiber in the diet. Sources of soluble fiber include oatmeal or oat cereal, lentils, apples, oranges, pears, strawberries, nuts, beans, dried peas, blueberries, psyllium, cucumbers, celery, and carrots.

-Weight reduction

-Increased physical activity of atleast 20 min a day. It can include walk, yoga, gym , swimming, cycling, etc. Swimming is especially beneficial to burn the extra fat.
