Hello. Thank you for writing to us
I am dr. kakkar(dermatologist). I have gone through your query and I have viewed the Images.
Unfortunately the images are blurred. I request you to kindly upload clear Images with better illumination.
The bump seems excoriated. This bump most likely could be an Ingrown hair Or probably a skin lesion of Molluscum contagiosum.
How long since you have it?Do you have any symptoms like pain?Do you have more such lesions?
Regards

Brief Answer:
Regarding molluscum
Detailed Answer:
Hi.
Although Genital Molluscum can be transmitted sexually but it does not usually happen after oral sex. Vaginal or anal sex is the usually route of transmission of genital molluscum. Anyways, Molluscum can happen even in sexually inactive individuals. Moreover you have a lesion on thigh which is not exactly genital area!

Dr. Kakkar. Do you suspect this is conclusive molluscum
Brief Answer:
Molluscum Contagiosum
Detailed Answer:
Hi.
Yes, it seems like molluscum contagiosum. But judging by the location of molluscum on thigh I dont think it would have been transmitted through oral sexual contact.

Dr. Can please give me advice
1- can i have sex unprotected sex with my wife? I have done hiv test last was last week (5months after exposure.
2- if so, should i cover the bumps with something so it wont spread to her?
3- how should i break this news to her?
4- can it be molluscum if the distance from old bump to new one is 20cm apart? The other suspected bump is on the other thigh
5- what other advice/screening of test needed for this? Or if i remove these bumps thru cryo or other means, can i have sex with her immediately after removal?
6- how many days usually the pus will develop? Is pus always indicative of molluscum?
7- should i do another screening for hiv?
8- should i tape the bumps so it wont spreas to my genita?
I dont know how to react to this now. Its 2am here and dermatologists are all closed and im having panic attack

Brief Answer:
Regarding molluscum
Detailed Answer:
Hi.
1) It would be better to get these molluscum lesions treated before engaging in sexual activity. Treatment is simple and it would hardly take a few minutes. Your HIV test is conclusive and no need to repeat it.
2) If oral sex was the only sexual encounter outside of marriage I don't think this lesion was sexually acquired from that encounter. As I already said Genital Molluscum is most acquired through vaginal or anal sex during which there is close skin to skin contact.
3) Yes, molluscum lesions can happen on a wide area.
4) Electrocautery or cryotherapy are both good options. Yes you can engage in sexual activity immediately after removal the procedure.
5) Since you have already tested for HIV. You may also screen for VDRL and HSV. No other test required.
6) Molluscum lesions do not contain pus unless there is bacterial infection. It contains molluscum body which is a semisolid structure rather than liquid like pus.
7) No more HIV testing required. Your tests are conclusive.
8) I suggest removal rather than covering with tape.
Regards
