Brief Answer:
Considerations

Detailed Answer:
Hello and thanks for the query.

This fluctuation of your heart rate should be assessed by your primary care doctor. Its possible that you may need a higher dose of the metoprolol to improve the control of your heart rate.

Your doctor may also want to request an EKG to make sure there are no other irregularities related to your heart rhythm as well.

Nicotine is a stimulant so please try to do your best to decrease or quit smoking in the near future.

Otherwise I suggest you see your doctor for a focused consultation and physical exam.

Sincerely,

Dr. Galamaga
