default
Posted on Fri, 10 Oct 2014
Question: I have a red rash on my scrotum and base of my penis. started on penis, I think. First noticed a few days ago. bumps that then become scabs. I used neosporin a few hours ago. I think I see more scabbing now.

I ride my bike a lot.

No recent sexual activity.

I took a couple of images, but can't figure out how to attach them.
This just in. Red rash is on one butt cheek, too. Not sure it's related. No bumps. Image on request

Brief Answer:
Take treatment as advised

Detailed Answer:
Hi again

I have seen the attached pictures.
The scabs over the penis surely look like a healing episode of herpes infection .
The rest of the rash looks allergic in nature.
Now most importantly, you need to stop applying everything over it. No dettol , oil or rash creams.
Simply clean it all with lukewarm water and apply a topical antibiotic cream like mupirocin over the penile lesions twice a day .
For the rest of the rash, mix and apply mupirocin cream with cortisone cream .
Take some oral anti histamine tablet like Claritin once a day along.

Hope it helps

Hi Dr XXXXXXX
I have never had herpes. And I think it's unlikely the two areas are unrelated. Agreed?
Some of the symptom checkers suggested MCV, fungus, folliculitis, bacterial or yeast infection.
What do you think?

Brief Answer:
Have reviewed the pictures,kindly follow

Detailed Answer:
Hi again

I have reviewed the pictures.
You should follow up the treatment as told earlier.
O understand that you have never had herpes infection before, but this time the lesions over the Penske region surely look like a mild self healing episode of heroes only.
Niw I assume that the lesions over the penis must have appeared first .
The rest of the skin rash basically looks irritant or allergic in nature. This could have happened either due to application of some cream or oil or medicine over the groin area which have reacted or it could also be due to sweating or friction.
I am sure the advised treatment would help you.

OK
I also have tenderness where left leg meets groin
Lymph node?
doctor
Answered by Dr. Geetika Paul (1 hour later)
Brief Answer:
Lymph Node swelling ,take anti inflammatory agent

Detailed Answer:
Yes

Such s tenderness can be due to swollen and inflamed lymph nodes. The inguinal lymph nodes can become swollen in case of any leg infections or in any infections over the groin region.
You can take some anti inflammatory agent like paracetamol for two to three days. Also as your groin problem settles, the lymph node will settle too.

Thanks. Does that indicate the type of infection?

The scrotum bumps are indented in the center, with scabs. Does that help with diagnosis?

The area on my buttocks looks like small blisters today. Image attached.
Does that tell you anything?

Thanks!

Brief Answer:
Need to look into more details

Detailed Answer:
Hi
Since new blisters are developing and the problem is increasing , I would like to get certain more details from you.
Are you applying any antiseptic liquid or oil or cream or using any specific soap or cleanser to clean the groin area?
Also please tell if the blistering is occurring only over one side of the body which is either on right side or left ? Have you noticed any unilateral pattern if all the lesions?
Also how much is the associated pain or itching or discomfort?

It's only on the left side of my body. What does that mean?I have been using Neosporin antibiotic/pain relief. They old bumps are scabbing over. Not much itching/pain.

The bumps appear to have a dark, indented center.

Brief Answer:
Seems like zoster infection , local treatment

Detailed Answer:
Ok.
So you have probably developed shingles or herpes zoster.
It basically affects a particular dermatome and the lesions are present mostly on one side of the body only in the affected areas.
Now since the lesions have started to scAb in your case, the only thing you need to do is to keep the affected area clean and apply an antibiotic cream like neosporin over the lesions twice a day to prevent any super infection .
Since shingles is a viral infection, antiviral drugs surely have a role in it but these are to be started within 24 to 48 hours of the initiation of the problem for maximum benefit.
So I don't think they are required in your case now .
The only thing is that if the lesions tend to persist or increase , then you should surely see your local doctor .

Hi Dr. XXXXXXX
Thank you very much. I had a shingles vaccine a year or two ago. Also, I'm getting new lesions that have not popped or scabbed. Small clear ones. And some with a dark dot in the indented center. Neosporin all around?

What's the prognosis? How long will it list, and will it return?

Answered by Dr. Geetika Paul (5 minutes later)
Brief Answer:
Zoster takes time to heal

Detailed Answer:
Hi again

Shingles vaccine does help since it makes the viral outbreak episode a lot milder than what it would have been otherwise.
But since you are still developing new blisters, the best thin would be to see a local doctor for a prescription of oral antivirals after complete assessment.
Herpes zoster has its own course which may take atleast ten to fourteen days for complete healing and may even take longer.
