Question: I TOOK A PRE- EMPLOYMENT PHYSICAL and egk results have to be seen by my doctor can you tell me if this can be explained in a manner that will allow employment ?


P/PR 128/156 MS
QRS: 88 MS
QT/QTc 400/413 ms
P/QRS/T Axis 36/33/7 DEG
Heart Rate 64 BPM

sinus rhythm
possible LVH
AGE CORRECTED SOKOLOW INDEX (SV1+RV5 OR V6) = 3.5 MV
AGE-CORRECTED R IN LEFT PRECORDIAL LEADS = 2.6 MV
moderate left precardrdial repolarization disturbance secondary to lvh, consider also ischemia
negative T in V5 V6

I am Dr. Saddiq Ul Abidin. I have read your problem in detail and will try to help you in best way possible.

Your EKG is suggesting mild changes which are normally seen in patients with hypertension. Left heart has to pump harder against high blood pressures in blood vessels, so like other muscles increased usage increases muscle size like you see in body builders' biceps. Also you need to get examined by your GP for any murmur which can lead to this. But rest assured, it is not acute emergency.

You need to screen yourself for high blood pressure, on follow up visits and keep it below 140/90 with life style modification, salt restriction, brisk exercise, and if all these fail to keep it that way, then antihypertensive drugs after discussing with your GP.

The other changes mentioned relating to ischaemia, as T wave inversions, in few chest leads, for that you have to relate with history of chest pain if any, and 2D Echo and blood tests for cardiac enzymes will rule out any such cause. It can also be a normal variant.

I do want to share with you that I am 5' 8 inches or 172.5 cm height and have exercised to take of weight from 239-225 pounds with a goal of an additional 35 pounds to reach 190 pounds of body weight.

that the since April 2015 during this time I have consumed 6-8 cups of coffee - no sugar- daily to try to assist with fat loss

and have used a electronic cigarette with a high nicotine content daily and numerous hours through the day

also using a testosterone transdermal system each 32cm patch contains 9.7 mg testosterone nominal delivery of 2 mg daily

could this also lead to the abnormal ekg ?

I am afraid that I have to say this but all the substances you have mentioned, have their harmful implications directly or indirectly on the heart.

Caffiene is directly associated with arrhythmias, and use in such excess is bad for heart rythm.

Testosterone and nicotine are associated with risks of thromboembolism leading to heart attacks.

I would insist you to discuss their usage with your cardiologist in detail and use these in controlled amounts according to his advise.
