Brief Answer:
You have Asthenozoospermia and Necrospermia

Detailed Answer:
Hello
Thanks for query .
Based on the reports of your semen analysis that you have posted You have what is called as Asthenozoospermia with Necrospermia .

It is primary goal to find out the cause for Necrospermia .

Ideally there are no medications that have been proved effective in treatment of Asthenospermia .With this count and dead sperms you are unlikely to impregnate your wife by natural means

You need to consult qualified Infertility specialist for clinical assessment and to undergo various blood tests and semen tests to determine the cause of dead sperms. You can discuss with him about the possibilities of conception with the help of newly assisted reproductive techniques like IVF or ICSI.

Hope I have answered your query ,please feel free to ask if you have more questions , I shall be happy to help you .
Thanks and Regards.
