Question: my sister is 51 years old and she is a heavy smoker. she suffered lately difficulty of breath when laid on her back and that was associated at first with some pain in the left jaw , arm and chest. her heart rate was 60 PBM and MP was as slow as 100/70 though she used to be a hypertensive.



Echo did not indicate any abnormality except for a mild Aortic Sclerosis and then the Chest X ray gave the following noted:



- Prominent central hilar vascular shadows with accentuated basal lung markings



- Dilated unfolded aorta with prominent aortic knuckle



is this an indicated of serious condition?

I can understand your concern.
Since her echo is normal, no need to worry for major heart diseases.
Smokers are at risk of bronchitis. And bronchitis causes similar kind of x ray picture.
So better to consult pulmonologist and get done
1. Clinical examination of respiratory system
2. PFT (Pulmonary Function Test).
PFT is must for the diagnosis of bronchitis. It will also tell you about severity of the disease and treatment of bronchitis is based on severity only.
Inhaled bronchodilators and inhaled corticosteroid (ICS) are the mainstay of treatment.
Smoking cessation should be done as soon as possible. With out this, no point in giving treatment.
So consult pulmonologist and discuss all these.
