What Does The Following Dental X Ray Report Indicate?

Question: My daughter had a xray with a new dentist today. This is her first visit in 2 years as we moved and I had a baby and hadn't gotten her in to a new dentist yet. He showed me a shadow like spot under her front and left front teeth. Her asked if she had had any trauma there or decay in the baby teeth to which the answers were no. He requested the xrays from her previous dentist and said he would probably refer her to an oral surgeon but wanted to see the previous xrays first. She has no swelling, pain, or any other symptom besides this spot in the xray. What are possible reasons for this and is cancer possible as he did mention biopsy. She is 12 and has no prior issues, not even a cavity. Thank you

1. The radio dense opacity in your x-ray - most possible cause could be old treated infection, after healing its fibrosis / calcification as your doctor suspect.

2. Next possible cause could be an hemangioma - blood based lesion, appears similarly on the x-ray, Other causes could be focal sclerosis of the bone ( no treatment needed )

3.Since there is no obvious cause you have known and no symptoms are been noticed. There is no evade of adjacent collection - which will suggest active process.
Further CT scan can be done - but i would recommend to do a followup x-ray after 6 months to see any change ( depending on that any action can be taken)

4.I would recommend a keen followup - there are no malignant features in the density ( cancerous). You don't have to worry about that.

5.Suggest a followup - if symptomatic immediate action can be taken.

I appreciate your answer. A few followup points to your response-

1. She has never had an infection so this doesn't seem like a possible cause.

2. How is this diagnosed/determined?

3. What do you mean by "no evade of adjacent collection - which will suggest active process"?

4. So you can tell it's not cancerous by the X-ray?

1.In that case the old treated infection will be ruled out.

2. Now the possible cause could be hemangioma of the mandible, which can be there for years but do not cause any effect on the adjeceg structures. Which needs followup x-ray.

3.No evidence of collection means - there is no active process adjacent / surrounding to the density.

4. Yes, the features are more to wards non cancerous type. ( the margins, rate of progression, bony destruction, adjacent reaction , involvement of soft tissue )

5. So, Mostly i would think followup/observation is a better option than intervening the lesion at this time. Or further investigations like CT - will give better evaluation of the lesion. MRI - will be good in bone marrow study.
