Brief Answer:
Blood in urine due to pressure of enlarged prostate on small blood vessels

Detailed Answer:
Hi XXXXXX,
Thanks for writing in to us.

I have read through your query in detail.
Your prostate gland is showing significant enlargement in size. It measures 64 grams against a normal acceptable weight of 25 grams. This is causing obstruction in your urinary passage. An enlarged prostate, also called benign prostatic hyperplasia, pinches the urethra and can lead to hematospermia or hematuria blood in semen or urine). This happens when the enlarged prostate causes pressure on the nearby blood vessels can dilate and rupture. It is possible that during sex, the small vessels in the urethra might have ruptured due the pre existing prostate enlargement and this caused little bleeding. This is not related to andropause.

Your PSA levels are in normal range and not a concern.

Please follow up with your urologist for complete clinical examination and treatment.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Brief Answer:
Avoid excessive straining during sex and at urination

Detailed Answer:
Hi XXXXXX,
Thanks for writing back with an update.

Your symptoms are due to stress on the small blood vessels by the prostate. During sex, the stress increased and caused slight injury. These will heal with time and to prevent it from happening again, please avoid rough sex and do not strain much during sex.

Food and drinks consumption is not related to your symptoms. Drinking more water will not cause any benefits, the injury will heal in a few days time. You can continue drinking the usual amount of water and this does not matter. Please avoid excessive straining at urination as it can cause further episodes of bleeding.

Hope your query is answered.

Brief Answer:
Please find details below

Detailed Answer:
Hi XXXXXX,
Thanks for writing back with an update.

Please find the impression mentioned in your report explained below in simple language.

1. no evidence of LITHIASES in the kidneys and urinary blader

This indicates there is no stone in your kidneys.

2. chronic bladder outlet obstruction versus CYSTITIS

This means that due to obstruction of flow of urine, probably due to the enlarged prostate, there is a slight infection in your urinary bladder.

3. prostatomegaly with HETEROGENOUS ATTENUATION and minmal CALCIFICATIONS, please correlate with psa and transrectal ulrasound correlation

The enlargement of prostate gland is described in this line. Further comparison with ultrasound scan and PSA levels is suggested.

4. minimal ATHEOROSCLEROTIC changes in the abdominal AORTA

This talks about the age related changes in your aorta, which supplies blood from the heart to the rest of the organs.

Hope your query is answered.

Brief Answer:
Annual rise in PSA and free PSA are important in those who have cancer risk

Detailed Answer:
Hi XXXXXXX,
Welcome and thanks for writing back with an update.

PSA levels are important to assess the risk of a prostate malignancy. Other than a single result, PSA levels done every year is advised in those who have a risk of prostate malignancy. This includes those having borderline high normal values and in those with family history of prostate cancer. Another test is free PSA levels. The amount of PSA in the blood that is “free” (not bound to other proteins) divided by the total amount of PSA (free plus bound). Some evidence suggests that a lower proportion of free PSA may be associated with more aggressive cancer.

Finally a diagnosis is made by prostate biopsy in those with suspicious imaging findings and PSA levels.

Please note that there is no risk of cancer in you if you have a confirmed benign prostate enlargement by imaging and PSA levels.

The normal fasting blood sugar levels is 3.9 to 5.5 mmols/l (70 to 100 mg/dl). In your case a fasting glucose exceeding 7.59 mmol/L will not cause significant problem on the healing but the raised sugar levels will delay any infections in the urinary bladder should you have it.

Hope your query is answered.

You mentioned possible infection on the bladder, would you suggest
anti biotic? to eliminate the possibility?. any suggested first aid medication?
as ive said it is still holiday here and the doctor is out until jan5 (monday)

Answered by Dr. Vivek Chail (3 hours later)
Brief Answer:
Please do urine culture and sensitivity test and take required antibiotics

Detailed Answer:
Hi XXXXXX,
Thanks for writing back with an update.

Antibiotics might be required for your infection in the bladder. If your doctor is unavailable right now then please get urine culture and sensitivity test done to know which bacteria is causing infection and what medication will work best in your case. Taking any antibiotic might not cure you and the infection will progress further and give you complications. Once urine culture and sensitivity report is known then you can take only the required antibiotics in recommended doses.

For first aid syrup containing disodium citrate may be used in patients to alleviate discomfort from urinary tract infections. It is taken as 15 ml mixed in a glass of water about twice a day. You should drink adequate amounts of water and visit washrooms regularly.

Hope your query is answered.

Brief Answer:
Please consult a surgeon for emergency care

Detailed Answer:
Hi XXXXXXX,
Thanks for writing back with an update.

Good morning.

It is important that you either call the urologist who you will be meeting on 4th XXXXXXX or go to your hospital and take emergency consultation from a surgeon. They will decide further tests and care depending on the amount of blood in urine. If serious then it might require cystoscopy and catheterization till you meet your urologist on Monday.

Brief Answer:
Urine reports confirm presence of red blood cells

Detailed Answer:
Hi XXXXXX,
Thanks for writing back with an update.

Guess your bleeding has stopped.
Urine reports show many red blood cells and this confirms that there is some focus of bleed in your urinary passage. Please continue drinking adequate water about 1.5 to 2 liters a day and pass urine frequently. Please do not strain while urinating.

Kindly see your doctor earliest and get the point of bleeding treated.

Answered by Dr. Vivek Chail (27 minutes later)
Brief Answer:
Your medications are correct and should help control bleeding

Detailed Answer:
Hi XXXXXX,
Thanks for writing back with an update.

The medication tranexamic acid is given to stop bleeding in the urinary tract. Tamsulosin works on the prostate by reducing its size and preventing pressure effects on an enlarged prostate. The treatment being given to you is right and should help control the bleeding.

The blood pressure is slightly above normal and I suggest you to remain free from any kind of stress or anxiety. If you have been given medications for high blood pressure then please continue taking it.

We also have urologist doctors at www.healthcaremagic.com who will be glad to help you out should you have any queries for them and wish it to be answered by urologists.
