What Does This NT Scan Report Indicate?

Question: Hello Doctor,

I already asked a question related to wife's pregnancy some time back.

My wife is now 15 weeks pregnant.
We have been consulting a specialist Doctor (Gynaec/Obs) for my wife's pregnancy.

As prescribed by doctor, my wife has a NT scan done on 21-Nov-2016 (at 12 weeks of pregnancy). NT scan report has been uploaded for your review.

NT Scan reported "Placenta is posterior and low lying".

On 03-Dec-2016 we again visited the doctor for consultation on NT Scan report.
Doctor had advised for 30 days of rest and asked my wife not to go to office for next 2 weeks (my wife is working in HDFC bank).
Doctor had advised for a USG Obstetrician for Placental localisation after 15 days.

Below are the medications details prescribed by the doctor for my wife.

1. TAB DOXINATE 1-0-1 (continuing for last 2 months, now doctor has asked to stop this medicine from 03-Dec-2016)

2. TAB MVISTA 1-0-0 (continuing for last 2 months and still going on)

3. TAB DUPHASTON 1-0-1 (continuing for last 2 months and still going on)

4. TAB OROFER-XT 0-1-0 (started from 03-Dec-2016 onwards)

5. TAB ACUCAL 1-0-1 (started from 03-Dec-2016 onwards)

My wife has been taking the medicines regularly as prescribed by the doctor.

We would like to get opinion regarding above medications, about the findings in the NT scan report and the way forward to go about the pregnancy (if some special care or treatment is needed etc.)

Note: From the time my wife has started taking TAB OROFER-XT & TAB ACUCAL, she saying that the colour of her stool has turned almost black. Is this normal?

We would really appreciate if you can provide your valuable feedback on above queries.
You can also call me on my XXXXXXX number +91 0000 or can provide your contact details so that i can call you to discuss things in detail.

Brief Answer:
Nothing to worry

Detailed Answer:
Hello sir, I understand your anxiety about the issue
Blackening of stools is common after iron intake (orofer XT) which is due to excretion of iron in stool.
Low lying placenta is seen in around 5 % of pregnancies but migrated to its original position by last month in pregnancy.
So in my opinion she can continue her job but just take care of not doing heavy work or intercourse.
Just don't panic and get a review ultrasound at 32 weeks
All the best

Hello Doctor,

Thanks for your reply.

Just few more confirmations.

As per your reply, you have suggested review ultrasound to be done at 32 weeks.

But our Doctor has advised for a USG Obstetrician for confirming Placental localisation after 15 days of rest(i.e at 17th week), is it necessary ?
Doctor has also advised for an Anamoly scan at 20-22 weeks of pregnancy.

Are so many ultrasound scans generally done ?
Are there any side effects by having so many ultrasounds, by now doctor has already made us undergone 3 Ultrasound scans.
1. Confirming intra uterine pregnancy gestational sac at 6 weeks
2. Checking heartbeat at 8 weeks
3. NT Scan at 12 weeks

Just one confirmation till what time generally below medications which are prescribed by doctor needs to be continued?

1. TAB MVISTA 1-0-0 (continuing for last 2 months and still going on)
2. TAB DUPHASTON 1-0-1 (continuing for last 2 months and still going on)
3. TAB OROFER-XT 0-1-0 (started from 03-Dec-2016 onwards)
4. TAB ACUCAL 1-0-1 (started from 03-Dec-2016 onwards)

Your answers are really appreciated.

Brief Answer:
Get anomaly scan after 15 days

Detailed Answer:
Sorry for the delay in response dear as I got struck in some emergency.
An ultrasound after 15 days will not have any difference in placental location so just get an anomaly scan after 15 days which was scheduled at 20-22 weeks.
International recommendations are to get anomaly scan at 16-18 weeks and in case (10% chances) there is any migration of placenta upwards then it will be another good point besides anomaly scan.
Ultrasound is safe in pregnancy so donot worry about it as far as number is concerned.
Duphaston I usually stop by 32 weeks and rest all will continue till delivery.
Wish her all the best and a healthy pregnancy ahead.
