What Does This Ultrasound Report Indicate?

Question: I am 4 months postpartum and have had bleeding/spotting since ive given birth. I have had an ultrasound that showed no retained placenta and also have had bloodwork that shows my hcg at less than 1. I have had one strange very light period and then started what i thought was another one two days ago. (26 days between these "periods") I have had episodes of heavy bleeding with clots since yesterday. I started on oral birth control yesterday to try to regulate my cycle and the bleeding stopped for most of the day today. About an hour ago i had another big gush of blood with some clots (theyre all smaller than a quarter and irregularly shaped).
I have a large fibroid as well.
Does this sound like a very heavy period? Or is something else going on? How much blood loss is considered an emergency?

Hi dear, I have gone through your question and understand your concerns.
Periods are irregular for few months after childbirth, due to hormonal changes in the body and the uterus regaining its original size.
Usually the first proper periods after childbirth, whenever they occur, are heavy with passage of clots. In your case, the fibroid can also lead to heavy periods.
Bleeding is considered alarming and needs emergency treatment, if the clots are bigger in size , you are soaking more than2 pads every2 hours, feeling dizzy or have low blood pressure.
If none of these symptoms are present, then you need not worry as it is normal to have a bit heavy flow in the first proper periods after childbirth.

Will the oral birth control pills eventually slow the flow?
What would cause the bleeding to completely stop and then start again?

Hi dear,
Oral birth control pills are known to decrease the flow of blood during periods and eventually stop the periods, due to high hormonal content they have.
Bleeding might have almost stopped due to oral contraceptive intake, and then recurred again when the effect of hormone started waning off in the body.
I will suggest you to take2 pills of oral contraceptives for at least3-4 days. This will help in decreasing and eventually stopping the blood flow. Following this, you can continue with one pill a day for next17 days to regulate the menstrual cycle.

For 2 pads every 2 hours, does that mean regular pads? Im wearing huge overnight ones so its hard to tell if its too much bleeding

Hello, i am on day 6 of bleeding still, but it has slowed down to a regular heavy period now. My last question is about the birth control. I just read that people who get migraines with aura should not take combination birth control. I get these. I also think the pills may be giving me anxiety. Would you recommend stopping them now? Or finishing the pack? If i stop them now, will it cause even more bleeding? I will follow up with my dr on Monday, but im not sure what to do in the meantime

Hi dear, people with migraine with aura should not take the oral contraceptive pills, as it leads to aggravation of migraine and can lead to anxiety.
You should stop taking the pills, however stopping them can lead to heavier periods.
I will suggest you to stop taking the pills, and take tranexemic acid tablets two to three times a day to decrease the flow of bleeding.
