Question: Hi, I've been having hematospermia for the past 2 weeks or so, but it doesn't always happen, sometimes it will be normal ejaculate, and sometimes there will be normal sperm, but blood will follow in the next urination. Blood tests and urine tests are fine. What do I do?

Brief Answer:
Need detail evaluation.

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that you have
Haematospermia since very recently.
I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you masturbate very frequently.?
2)Do you have unprotected sex with unknown woman preceding having haematospermia.?
3) Do you have any urinary symptoms ?
4) Do you have pain in perineal region associated with fever?

Haematospermia at young is mostly due to infection and inflammation of the organs involved in production and transmission of semen.(Epididymis,Vas difference,Seminal vesicle , prostate and urethra and trivial injury due to rough handling of penis while masturbating, and needs to be investigated to find out the source of bleeding.

Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests to confirm the diagnosis.
1) Routine urine test and culture
2) Semen examination and culture.
3) Ultrasound Scanning of the urinary system and scrotum.

He may ask for additional investigative test like MRI,Cystoscopy, depending upon clinical findings.

In the mean while start taking broad spectrum antibiotic like Cefexine along with anti inflammatory drug like Diclofenac twice daily.

Hope I have answered your query ,please feel free to ask if you have more questions ,
I shall be happy to help you
Thanks and Regards.

Hi doc,

1)I masturbate/have ejaculatory sexual activity about once every two days or so.
2)I have never had sex without a condom, ever.
3)No urinary symptoms, but sometimes blood does not come out in the ejaculate, but appears in urine passing minutes later.
4)No pain and no fever.

I have already taken the urine culture and test, with no infection present. Blood tests have also been taking with no worrying results.

I am taking trimethoprim as an antibiotic.

Sometimes, there will be no blood at all, in sperm or in the urine after. I notice that the more vigorous the masturbation/sex, the more likely for blood to be present.

My local doctor requested i take a CT scan, but this is extremely expensive and decided to ask here first, to see what is my best course of action.

Thanks for your help,

doctor

Brief Answer:
Seems to be due trivial injury to urethrhra.

Detailed Answer:
Hello
Thanks for follow up.
Haematospermia in your case seems to be due to trivial injury to urethra resulting out of rough handling and vigorous shaking of penis while masturbating.

Please continue medicines prescribed by your Dr.and take anti inflammatory medicine like Diclofenac twice daily along with antibiotic.
Thanks and Regards.

Hi,

But if its injury to the urethra, how come it doesn't bleed into normal urine?
Should i stop all sexual activity to avoid more inflammation?
Should i take the CT scan?

Thanks

Brief Answer:
Stop sexual activities for time being.\

Detailed Answer:
Hello
Thanks for follow up.
The trivial injury is minimal injury which bleeds is slight bleeding immediately and blood gets mixed in semen hence may not be seen in urine.

Refrain from sexual activities for couple of days and observe for any improvement.
Till then you can withheld C.T scan.
Thanks and Regards,
Dr.Patil.
