Question: I WAS TOLD THAT AFTER A ESCHEMIC STROKE, I AM TO TAKE
40MG OF ATORVASTATIN AND 75MG
OF CLOPIDOGEL FOR THE REST OF MY LIFE. MY LDL IS 129, WEIGHT IS 143, AGE 79 iN PERFECT HEALTH UNTIL THIS HAPPENED . THE ATTENDING PHYSICIAN SAID IT WAS CAUSED BY THE ESTERDIOL THAT I WAS TAKING.

dOES THIS SOUND LIKE SOUND ADVICE? I THOUGHT THAT PLAVIX WAS SOMETHING THAT WAS ONLY USED FOR A SHORT TIME. ESPECIALLY AT THAT DOSAGE. wHAT WOULD YOU ADVICE AS FAR AS DOSAGES AND LENGTH OF TIME THAT IT SHOULD BE TAKEN? WHAT KIND OF FOLLOW UP MONITORING SHOULD THERE BE WITH THESE DRUGS?

Hi,
Welcome.
This is Dr Sameer, cardiologist.
Ischaemic stroke is not just caused by estradiol, but it is caused by combination of variety of risk factors which also includes taking of estradiol. Initiating event is formation of cholesterol clot in the blood vessels which progress to block the whole vessel & cause stroke.
Treatment include taking of Blood thinner & cholesterol medication throughout life.
Blood thinner can be Aspirin or Clopidogrel which is given to prevent further clot formation & keep the blood vessel open.
Cholesterol medication can be Atorvastatin or Rosuvastatin, which keeps the cholesterol in range & also acts as antiinflammatory which prevent inflammation process in blood vessels.
Risk with blood thinner is bleeding which can be from any part of body. It occurs in very less number of patients but still it happens. So if their is any bleeding, you should report it your doctor.

I do want to know how often the effectiveness and functioning of these medication should be checked. Is it ever going to be possible to lower the dosage of either of these medications?
Clopidogrel 75 mg. and atorvastatin 40mg. What would be the indicators if this is possible?

Hi,
Welcome back.

There is no such test to check the effectiveness of these medications. The dosage & these medications were formulated after testing them on several patients for several years & they were found to prevent further clot formation.

You just have to see for bleeding which is a side effect & should report it to your physician.

There are combinations available for both these drugs & that can be taken. Further 75mg is the smallest effective dose of Clopidogrel & Atorvastatin can be reduced in dose to 20mg after a year or so as per physical assessment.
