16 Year Old With Shoulder Pain During Warm Up Exercise. Reason?

Hi,
I am a 16yr old baseball player
I've had shoulder pain all season
I warm up by running and stretching carefully
The pain is always present in my shoulder during the first 20 - 40 throws I make, no matter how hard
The pain has lead to a significant decrease in velocity
It is ONLY present during the "warming up" phase which seems to baffle me
The pain happens when the arm is fully cocked back and begins to whip forward...
I have iced and even taken 2 weeks off... nothing works so far... diagnosis?
