Hello,
You have primarily two issues.
1... Anemia
2... Breathlessness and weight on chest.

1... Cause of your anemia should be found out.You must be investigated for chronic kidney disease ( CKD ),as it leads to anemia.Get investigated for kidney function tests
2.. You must be investigated by a cardiologist to rule out cardiac involvement as your symptoms are suggesting cardiac problem.Relevant investigations like ECHO should be done as ECG is an unreliable test in diagnosing heart problem.Long standing diabetics have much more chances of developing cardiovascular complications.

Consult your doctor and get yourself examined thoroughly to find out the basic cause for our problem.
Thanks
