Abdominal Burning Pain Under Rib Cage Extending Upto Back In Right Side Usually After Meal. Ultrasound Normal. What Tests Should Be Done?

For six months I have had repented right sided abdomenal pain under the rib cage. The pain builds up with pressure to a peak than goes away. This happens after a meal. Eating pork chop has been the worst pain ever with vomiting. A ultra sound shows normal. I'm going into hospital for a scope soon, both upper and lower. Question to find what? The pain has only been under the rib cage. It has a burning feeling and extends up my back to. Help please.

Hello,
Thanks for the query to H.C.M. Forum.
As pain , you mentioned in the query is on the right side under rib cage and aggravated by pork chop means greasy food+ spicy food.
The first possibility is of peptic ulcer or Hiatus hernia.
Diagnosis can be confirmed by Upper G I endoscopy.
Try raising the head of your bed about 4 inches with blocks. It also might help to avoid eating or drinking for two hours before you lie down.
As fundus of stomach lies on the right side so may be a stomach pain.
Also avoid greasy or spicy or pork chop type food .
You can use antacid for some relief.
2nd reason may be thickened pleura under the right rib cage, so consult your physician and get in an X ray of this part.
3 rd reason may be disease in liver or gallbladder so ultrasound ( repeat) is important.
Good luck.

Hello,
Welcome to Healthcaremagic.

I had gone through your query and understand your concern.

It suspect you may have either Peptic ulcer or Gallstones.

As the pain is only under the rib cage, it is most probably due to gallstones.

But it is also associated with severe pain on taking pork chop, it may also be due to peptic ulcer or gall stones.

If you are my patient, i will start with proton pump inhibitors like Pantoprazole 40 mg once daily for 2 weeks.
At the same, i advise you to upper Gi endoscopy and H.Pylori test.

If H.Pylori is present, i will definitely add appropriate antibiotics like amoxicillin or clarithromycin depends upon the lesion in endoscopic findings and positivity of H.Pylori.

If there is no improvement, i will suggest you to go for liver function tests and fasting lipid profile.
Then i will try to manage accordingly with cholesterol level and liver enzymes . If possible i will start deoxycholic acid tablets.

Thus i suggest you to do endoscopy first and review with results.
Hope this helps.
If you have additional questions or follow up queries then please do not hesitate in writing to us. I will be happy to answer your queries.

Wishing you good health.

Regards,
Dr.Manjeth.
