Am I About To Have A Heart Attack

I m 39 years old. I have a painful tightness in my chest, my left shoulder hurts, the left side of my neck hurts, and my heart palpitates. I also have this feeling like acid reflux that makes me feel like coughing, I admit I don t take care of myself all that well (I drink quite a lot and eat fatty foods) and I exercise very little. I look fine on the outside (6 feet tall, 170 pounds). I just lost my job and have no insurance but I don t want to go to the ER and have them tell me I m fine, sticking me with a huge bill. I should also add that I have twins on the way and a troubled teenaged son. Stress, perhaps?
