High bilirubin without high transaminases (still better with a protrombine time normal and a protein electroforesis normal, if done) is not a situation of great concern for us hepatologists. Anywhere total bilirubin should be fractionated in direct and indirect and haematological cause of it should be taken under consideration, still more with a mild increased spleen.
