Are Fissures Chronic?

I am 27 year old female, had fissure from the age of 15. Did anal sphincterectomy for anal fissure with sentinel piles in 2010. I had a severe pain in dec 2013 and then got healed using 0.2 GTN ointment. For past one month had stopped using ointment and followed a strict diet everything worked out perfectly fine. Can eating spicy food one time can lead to opening of fissures. Now again I am facing fissure, as i had a spicy food last week and did a little bit of travelling. Is it chronic? My skin tag bothers me alot when i have fissure.

Thanks for your query.
Noted your history of being a female of 27 and have recurrent anal fissure for twelve years for which a surgery for sphincetrotomy and sentinel piles was done. But it recurred. Spicy food was taken.
The long standing nature and recurrence in spite of surgery indicate that you have some basic problems.
I would advise you the following.
Get a colonoscopy done. tests of stool and blood.
Diet restriction as over such a long period you must have noticed what is causing or precipitating this.
Stop all such foods and beverages that cause the problem.
Sitz bath.
Lord's dilation under general anesthesia may help a cure.
Avoid constipation.
And you may be fine.
No spicy food at all.
