In 5th week of pregnancy, bleeding with cramp is mostly suggestive of early sign of miscarriage. You may need to undergo one TVS scan and blood for beta HCG level. Early scan may not reveal detailed picture on feto-placental profile.
You may need to take hemostatic agent with micronised progesterone medication to avoid miscarriage & support pregnancy.
Complete bed rest with avoidance of heavy work/ uneven surface/ high heal shoe etc, take vitamin & folic acid supplements with healthy diet and maintain genital hygiene properly.
Be well and take care yourself.
