Risk is increased with more blood thinners. The combination of aspirin, Plavix, and Xarelto can greatly increase the risk for a bleed. Fish oil may also have a blood thinning effect. This combination of medications needs to be very closely monitored by a doctor. Does the doctor prescribing the Xarelto know about the other drugs? It might be best to go off at least one of the others. This should be discussed with a cardiologist who would best know the nature of the atrial fibrillation and other heart problems and the best combination of treatments for his condition. Hope this helps.
