Child Has Trouble Reading, Comprehending Colors. Suggest Referral Plan?

I have a 4 1/2 year old daughter named Callie and her preschool teacher said my daughter has these behaviors vocabulary of 3 to 5 words, uses her fingers to eat, doesn't recognize her written name, does not know her colors, and has difficulty following one and two step directions and she wants to have a referral plan made up for her. What do you think? And if you agree what would be your referral plan for her?

i understand your concern regarding daughter
4 1/2 year old should be able to talk atleast 200 to 300 words with sentence formations, stand and run , should be able to write letters properly.
sorry but your child seems to be lagging behind in these aspects
first consult pediatrician because he is the one who can examine the child properly and decide what is next step needed to be taken .
