Hi,
After going to his history it seems to have bronchospasm and inflamation of airways. There are few possibilities like Chronic bronchitis,Emphysema,Brobchial Asthma or Infection of lungs(Pneumonia).
Is he a smoker? or exposed to chronic fumes or chemicals or Obese . First one should find out is it COPD, Some thing else like heart disease causing cough or breathlessness problem. He should take a ECG,A plain X ray chest ,Pulmonary function test ,Rotine blood test like Complete blood count,Blood glucose and serum Creatinine. After which A CTScan chest,A 2 D Echocardiography and a Ultrasonography may done to find a cause and in smokers we should also keep a diagnosis of malignancy??. Treatment if there is no infection antibiotics hardly help but inhalation of bronchodialators and steroids should help him with nebulizer. For pain safe analgesics may be given.
