Could The Swelling In Hands Be Due To Rupture Of Blood Vessels?

My husband got his hand caught in a 60 quart mixer at work two days ago....we have tried to keep ice on it (20 on/20 off) but the Dr said he ruptured the blood vessels in the back of his hand. Today it's swollen twice as large and looks like it's turning a blue/grey. Should he go to the ER, I'm afraid the swelling is a bad sign since it's now swelling to the elbow and I'm worried about a blood clot.
