Ruling out a chronic fungal infection , secondary bacterial infection/ folliculitis ,cold sores and eczema would help.Tinea cruris or fungal infection is the most likely problem with a possible overlay of bacterial infection.Use of Doxycycline for 10 days along with oral and local antifungals will help cure this.the local application should be continued for 3 weeks after the symptoms have cleared to prevent recurrence.Use cotton underclothing, keeping the area dry will help in recovery.
