Orthopaedic Surgeon, Joint Replacement 's  Response
hi,
i understand your concern.
since there is no damage in the bone and shape is restored and also your sister is 25 years of age only, you can try medications. although the results of these medications is not very predictable. you can also go in for a surgery called CORE DECOMPRESSION WITH FIBULAR GRAFTING which is typically recommended for stage 1 and 2A. you can discuss this with your treating doctor.
although the cause of AVN in majority of cases is unknown one should try and eliminate certain risk factors associated with it e.g., smoking, long term steroid intake.
regards
