Diagnosed With PTTD. Had Bunionectomy, Surgically Removed Arthritic Material. Is Surgery The Only Option?
I have been diagnosed with PTTD. The doctor is not sure if the tendon is torn or if its simple tendonosis...I am scheduled for an MRI. I am a 63 year old athlete, and in very good condition (except for my left foot). I had a bunionectomy about a year and a half ago and the surgeon removed a considerable amount of arthritic material. I think the PTTD is possibly related to the earlier surgery (no orthotic was prescribed even though the mechanics of the foot were changed). My main concern now is fixing the PTTD as quickly and thoroughly as possible. If surgery is prescribed I want the best surgeon (most experience, best success, best after surgery care) available. The question is 1) what Kaiser dr has the best record with PTTD 2) if the tendon is torn, is surgery the only option.

Hi, thanks for writing to HCM. This is just to inform you that bunionectomy is in no way related to PTTD (posterior tibialis tendon dysfuntion). PTTD is the commonest cause of Acquired flat foot in adults which results in pain on weight bearing. Not all cases of PTTD needs surgical intervention. Conservative management includes support of the longitudinal arch by using an ankle-foot orthosis (AFO). If there is a rupture of the tendon, then surgical correction is inevitable. This includes reconstruction with FDL tendon. If other tarsal joints are also involved, then an ARTHRODESIS might be needed. Hope this information is helpful. Good day
