Does Pain In Anterior Shoulder Area Indicate Rotator Cuff Problem?

I have deep throbbing pain in my anterior shoulder area which radiates over my collarbone and sometimes up into my neck and ear. Sleeping on the afftected side makes it worse. Is this a rotator cuff problem. I am able to lift my arm above my head, does hurt to take a shirt off over my head though.
