This sounds like a trigger point, which is a knot of fibrous tissue within a muscle. It is usually tender to touch, and when manipulated can send pain radiating in different directions. It responds well to local injections of lidocaine, called trigger point injections. It can also respond to massage therapy, as well as acupuncture.
