Fever, Eruption On Neck Which Discharged Pus, Diagnosed With MRD TB Of Lymph Nodes. Treatment?

hi.my mother used to have constant fever and one day she saw an erruption on lower right side of her neck which bursted in a week and pus came out of it we got the pus tested and came to know that she has been suffering from mdr tb of lymph nodes for past 2-3 years.first she was given first line treatment for 5 months then she went to AIIMS delhi where she got a second line treatment for 2 years and now within two months after her treatment got over she has again got a kind of erruption on lower right side of her neck.please tell us what treatment should we take now and also where are we getting wrong.......

Thanks for coming to HCM
considering your mothers case diagnosed of having MDR and has taken proper second line treatment for 2 years...again there is eruption...
You must go for FNAC of the new eruption and see whether it is TB or not. If it is TB again go for culture and sensitivity for full first and second line drugs.
Many a times it happens that the TB gets cured but the new eruptios are there because of body reaction to mycobacterial protein.
In the meantime you must monitor for her weight and other cardinal symptoms of tb

If it TB and is MDR only again you may have to take treatment but if it is XDR i.e MDR plus resistance to any injectables like amikacin, kanamYcin or capreomycin and to any of FQ like levofloxacin or moxifloxacin then you have to take category % regimen....
