Full Term Neonate With Early Sepsis, Vomit Of A Clot Of Blood, Dicynone Any Help?

Hi ,I am a neonatologist and I have neonate ,who is full term, with an early sepsis , he was on a ventilator and weaned from it after 3 days ,suddenly he vomits a clot of blood ( hematemesis ,melema which soon became hematochesia),blood was given ,no lab was performed as it is an emergency right now,I aske about Dicynone could it help till cryo and platelet were tranfused (he is thrombocytopenic plt 60.000/cmm),

Hi , Dr Khaled,

Welcome to HCM.

You can definitely go for Dicynone ( Etamsylate) to halt the on going GI bleeding. I think you know the dose to be given in neonates ( 12.5mg/kg body weight every 6th hrly).

If the child is on ryle's tube, infusing cold normal saline though ryles's tube shall help in controlling GI bleeding in some cases ( can be tried in emergencies . Succesfully tried personally in few cases).If possible try to confirm whether the bleeding is of variceal or non variceal in origin. You can perform a bed side Ulatrsonogram.
